Assessment

Wellness is an active process of becoming aware of and making choices toward a healthy and fulfilling life. Wellness is more than being free from illness, it is a dynamic process of change and growth.

This Wellness Assessment will help you identify the dimensions of wellness (emotional, environmental, intellectual, occupational, physical, social and spiritual) in which you are excelling and the ones that may need improvements. Once you take the assessment and learn your results, re-visit the Wellness Assessment webpage to take advantage of the dimension specific local community resources that can help improve your overall wellness.

INSTRUCTIONS
1. Answer all the questions for each of the seven wellness dimensions.
2. Provide a valid email address and review your assessment scores provided via email.
3. Re-visit the Wellness Assessment web page to learn specific community resources for dimensions that may need improvement (lower scores).
