Community Health Assessment and Improvement Process
In 2018, the Jessamine County Health Department and the Jessamine County Healthy & Safe Communities Coalition conducted a community health assessment (CHA) and improvement process (CHIP) with input from our residents and community partners.
Although we have made great strides in the past five years, it is time to reassess our current health status as a community.
On September 26, during the CHA Community Partners Lunch-N-Learn, 37 public health systems partners came together to learn more about the community health improvement process journey. The underlying goal of this process is to strategically identify, through collaborations, the top health issues most pressing to our community, then devise a plan to improve these issues and build upon current improvement initiatives.
Community Health Conversations Forum
Our Community Health Conversations Forum was held Tuesday, November 28. 37 community partners reviewed and discussed community health data and shared insights on factors and pressing health issues impacting Jessamine County’s health status.
