Our community is committed to prioritizing prevention education, healthy and safe living environments, and mental wellness with a focus on informed choices, hope, and well-being for all.

Overview

Every five years, spearheaded by the Jessamine County Health Department (JCHD), the Jessamine Community Health Improvement Plan (JCHIP) Coalition engages the community in a health assessment and planning process for the purpose of improving health for everyone in Jessamine County. The 2023 -2024 Community Health Assessment and Improvement Planning (JC CHAIP) process for Jessamine County has been created to address the priority health outcomes and issues identified through extensive data collection, analysis, and partner collaboration. This process results in a community health assessment (CHA) and five year community health improvement plan (CHIP).

The following priority issues were identified during the JC CHAIP process:

  2. Social and Mental Wellness
